NOTICE OF OFFICE RELOCATION

Dear Patients

We are sending you this notice to announce that as of April 1, 2022, the location of our current practice will be relocated to a new, larger, location in Burlington to continue to provide superior primary healthcare to our valued patients.

Providing our patients with only the best possible care will continue be our goal. In the new location, there are many other medical services such as blood lab, x ray and ultrasound, walk in clinic and multiple medical specialists to provide you with one stop medical care. This location also will be with our other colleagues who have been in practice there for many years and have nothing but positive feedback.

Changes and transitions are always difficult, but the physicians and the staff will make the transition as smooth as possible for everyone. All your medical records will be transferred with your physician to maintain continuity of care. If you would prefer to transfer to another physician, please contact us and we will assist with the transfer of medical records.

Dear Patients,

As the situation surrounding COVID-19 continues to evolve, we want you to know that we are thinking of you.

During the COVID-19 Pandemic, all physicians at our office have been accepting virtual appointments over the phone and will continue to do so for the foreseeable future. In-person appointments have also been made available. Please only one medical concern to be discussed at a time, when booking for virtual appointment.

Please continue to call us for any health care concerns that you have, and we will be happy to schedule you for a virtual or in-person appointment.

As we all need to accept that we will now be living a new "normal", the highest level of health and safety standards will continue to be in place at our clinic. To prevent further spread of COVID-19, we have put additional precautionary measures in place such as patient screening for COVID-19 symptoms, staggered appointment times to limit clinic traffic, 6ft markers on the floor to help patients physically distance from each other and extra cleaning between visits. Patients are being taken directly into exam rooms upon arrival and hand sanitizer is also readily available upon entry to the clinic. Additionally, we are also working to install plexiglass barriers at the reception areas.

We are also asking all patients over the age of 2 to wear a non-medical face covering when coming into the clinic.

The safety of our patients, staff and physicians is our number one priority, and we sincerely hope that the measures we have put in place will make all patients feel safe and more at ease when visiting our clinic.

At a time like this, it is important to stay positive and look towards a brighter future when this is all in the past. But until that time, we must continue to work together to do our best to fight against the spread of COVID-19.

Please remember, WE ARE OPEN and providing virtual and in-person care to all patients.

If you require walk-in clinic services outside of our regular office hours, our walk-in clinic is also open 7 days a week for virtual appointments.
